Director of Adult Care
Neighborhood Health is seeking a Director of Home Care service. The director will be responsible for the operations and programs of a clinical nature within the agency. Directs, Coordinates, and supervises the managers of assigned nursing teams and/or therapists. May be responsible for a team of clinicians. Responsible for monitoring quality of care provisions, outcomes of care, job performance, direction of staff including evaluations and counseling staff, and financial operations of the homecare department.

Requirements:

  1. Master's in Clinical area such as Nursing or Physical Therapy
  2. MBA preferred
  3. Minimum of two years of home health preferred and minimum of five years of clinical experience
  4. Minimum of two years of supervisory experience
  5. Holds a valid license to practice in the state
  6. Computer literate

Hospice/Palliative Care Nurse Practitioner
Neighborhood Health is seeking a Per Diem Hospice/Palliative Care Nurse Practitioner. This position works in collaboration with the palliative care team members to deliver evidence based palliative care to appropriate patients and families. Proactive identification of opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resources use is essential. Advanced Practice Nurse experience necessary, Hospice or Palliative care experience preferred. EOE
