Sometimes a patient needs more care than can be managed at home, or symptoms require more intense management than a family caregiver can provide. At other times, a patient's support system may break down due to accident or illness. For these times, which are usually of short duration, five to seven days, Neighborhood Hospice has an inpatient unit. Inpatient services are usually reimbursed by Medicare or private insurance once specific clinical criteria are met.
Our Team - We're here to help
At Neighborhood Hospice Inpatient Unit, our highly skilled multidisciplinary healthcare team works together to care for people experiencing the final stage of life. Our hospice team is comprised of a medical director, nurses, physical therapists, social workers, spiritual care coordinators, certified nursing assistants, volunteers, behind-the-scenes support staff and your physician. Registered nurses and Certified Nursing Assistants are on duty 24 hours per day every day. Our social workers, spiritual care coordinators, and volunteers are available throughout the day.
Personalized care is designed around our patient and is mission focused, to provide comfort and dignity to our patients while caring for the emotional, spiritual, physical and psychosocial needs in a home-like environment. Admissions to and from the unit are coordinated with the patient's physicians and the Hospice team.
Our Care - Compassionate support for patients and families
Each member of our hospice team is dedicated to providing the highest standard of care. Our goals are:
