The Home Readiness Assessment provided by Neighborhood Health Agencies, Inc. (NHA), part of The Chester County Hospital and Health System, consists of an environmental analysis, a bilateral joint assessment and an exercise-technique instruction program, all performed by a licensed physical therapist. The therapist will assess the safety of a patient's home and the patient's readiness to return home following total joint replacement. NHA rehab staff will also evaluate the patient's physical strength and range of motion and provide instruction in proper exercise technique, to assist in personalizing the physical therapy care plan while hospitalized. This service is available for free for those patients that have pre-elected NHA as their preferred home care agency.
