Chronic Heart Failure Telemedicine Program

Chronic Heart Failure (CHF) affects more than five million Americans and is associated with frequent hospital admissions for worsening of heart failure symptoms.

Patients who understand their heart failure and who self-monitor with regular support from a nurse or other healthcare provider often feel better. They find that they require fewer visits to the hospital, and fewer unscheduled visits to the doctor's office for complications or worsening of their heart failure.

In addition to an inpatient heart failure education program, The CardioVascular Center offers a monitoring program for patients with CHF. We are pleased to offer our patients with heart failure an opportunity to participate in self-monitoring via telephone. There is no cost for this telemedicine service.

CHF NursesIf you choose to enroll in the telemedicine program, a Registered Nurse will work with you and your family to provide information about managing your heart failure. You will be asked to call a toll-free number at scheduled intervals to self-report weight and overall symptoms. Your nurse, in close collaboration with your doctor, will contact you to answer questions and to follow-up if you report symptoms that may be an indication of worsening heart failure.

The telemedicine service is intended to supplement and reinforce the services already provided by your doctor. It does not take the place of your regularly scheduled visits with your doctor.
