True to its mission of delivering care across the continuum, The CardioVascular Center at The Chester County Hospital participates in a federally funded Chronic Heart Failure (CHF) Disease Management Program. The Hospital has collaborated with Siemens Medical Solutions for several years to develop a telemedicine software program to help patients with congestive heart failure to monitor and control their disease better. This technological support coupled with the personalized care and education patients receive from the Heart Failure clinicians helps the patients feel more comfortable when they return home. The goal is to decrease unscheduled visits to physicians' offices and the Emergency Department, as well as unplanned hospitalizations, thus improving their quality of life. The Chester County Hospital's commitment to manage Chronic Heart Failure was recognized with a Gold Seal of Approval™ by the Joint Commission in 2008 and 2010.
